Consonus Pharmacy is a closed-door long-term care pharmacy specializing in the needs of nursing facilities, assisted living residents, and their caregivers. We are currently hiring a  Consultant Pharmacist/TOC Pharmacist to join our team!

As a Consultant Pharmacist on the Transitions of Care (TOC) Team, you’ll play a key role in ensuring safe, effective, and compliant medication management for residents in our partner communities. This position combines clinical expertise, regulatory knowledge, and collaboration with interdisciplinary teams to improve outcomes and support smooth care transitions.

Qualified candidates must hold—or be in the process of obtaining—pharmacist licensure in Oregon, California, Washington, Nevada, Minnesota and other states as requested by the Supervisor or Client Care leadership.
A flexible schedule, including some weekdays and weekends, is required to accommodate facility needs.
As the  Consultant/TOC Pharmacist your responsibilities will include, but are not limited to:

  • Perform admission medication regimen reviews, interim medication regimen reviews, comprehensive medication regimen reviews, medication regimen reviews, and disease state management reviews daily.
  • Conduct monthly or as-needed resident medication reviews based on acuity, providing written reports to facility leadership (Administrator, Director of Nursing, and prescribers).
  • Maintain a working knowledge of state and federal pharmacy regulations relevant to long-term care practice.
  • Plan and deliver in-service education for pharmacy staff and facility teams as requested.
  • Understand, implement, and uphold all applicable pharmacy policies and procedures.
  • Collaborate with medical directors, facility nursing leadership, and provider pharmacy staff to develop and maintain emergency drug supply at facility.
  • Serve as a liaison between facility and pharmacy operations teams to promote communication and service excellence.
  • Provide education to residents preparing for discharge, focusing on medication safety and adherence.
  • Demonstrate understanding of CMS regulations and interpretive guidelines.
  • Pursue ongoing professional development and continuing education relevant to practice.
  • Participate in quality assurance and interdisciplinary committee meetings as requested.
  • Prepare and submit quarterly quality assurance reports as required.
  • Other duties as assigned.

Qualifications:
  • PharmD or BSPharm degree from an accredited School of Pharmacy.
  • Current pharmacist license(s) in state(s) of practice.
  • Experience in long-term care, consulting, or related pharmacy practice preferred.
  • Valid, unrestricted driver’s license and ability to travel throughout assigned territory (some overnight travel required).
  • Strong clinical pharmacy skills and sound clinical judgment.
  • Excellent verbal and written communication skills.
  • Proven ability to collaborate within an interdisciplinary care team.
  • Proficiency in learning new technology applications.
  • Strong organizational skills and ability to manage time effectively.
  • Self-directed and able to work independently.
  • Flexible 7-day availability to accommodate facility requests.
