The overall responsibilities of the Case Manager include facilitating the achievement of client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Based on the needs and values of the client, and in collaboration with all service providers, the case manager links patients with appropriate providers and resources throughout the continuum of health and human services and care settings, while ensuring that the care provided is safe, effective, client-centered, timely, efficient and equitable. This approach achieves optimum value and desirable outcomes for all – the patients, their support systems, the providers, and the payers. This position reports to the Director of Case Management.
Position Qualifications:
Must be graduate of accredited nursing program and be a currently licensed, registered nurse (RN)
OR
Must be graduate of accredited nursing program and be a currently licensed practical nurse (LPN) in the State of Illinois.
Must have a minimum of two years healthcare experience
